If you take a step back and look at the engineering of it, the traditional approach is actually quite messy.

Here is the situation. Globally, one in five adults is stuck in a cycle of chronic pain. In the US alone, that is a hundred million people. But the numbers don’t tell you what it feels like. It feels like waking up tired, going to work distracted, and coming home exhausted.

For decades, the standard answer to this specific mechanical problem has been a general chemical solution. We give patients pills. But pills are not smart. They do not know where your back hurts. They just flood the entire system. They go to the brain, the stomach, the liver. They make the patient drowsy, they shut down digestion, and often, they lead to a dependency that ruins lives.

The Fire: Precision Through Heat

Let’s look at the first half of this equation. This is the LG2 Radiofrequency system.

The concept here is friction. When you pass a high-frequency current, specifically 488 kHz with this system, through tissue, the ions in that tissue start to vibrate.

With the LG2, we can control that heat with incredible precision. We place a probe right next to the sensory nerve that is broadcasting the pain signal. We raise the temperature to the required level, typically between 60 and 90 degrees Celsius, utilizing the system’s full range of 30°C to 95°C. This heat denatures the proteins in the nerve.

Think of it like a telegraph wire that is stuck in the “ON” position. We are simply cutting the connection. The signal stops. The pain stops.

For clinic owners, the beauty of the LG2 system is efficiency. It has two independent channels, meaning you can treat two sites at once. But more importantly, it has a safety feature called Neutral Electrode Monitoring (NEM). It constantly watches the electrical return path to ensure the patient is safe. The Ice: The Thermodynamics of Repair

Then, we have the other side of the coin. The Ice.

This is the C3 CryoSystem, and this is where the physics gets really fun. This device uses the Joule-Thomson effect. If you have ever sprayed a can of compressed air and felt the can get cold, you have felt this effect. When a gas expands quickly, it pulls heat out of the environment.

The C3 uses this principle to freeze the tip of a probe down to approximately -55°C to -60°C. When we apply this cold to a nerve, we aren’t burning it. We are putting it into a state of suspended animation.

Here is why we at Sonna get excited about this part. When you freeze a nerve, you induce something called reversible axonotmesis.

Imagine the nerve is a copper wire running inside a plastic insulation tube. The Fire (RF) melts the wire and the tube. But the Ice? The Ice stops the wire from working, but it leaves the insulation: the connective tissue, completely intact.

Because that structure is still there, the nerve has a scaffold. It can regenerate. It grows back along the original track at a rate of about 1mm to 2mm a day. This means you get immediate pain relief, but you also allow the body to heal itself naturally over time. It is the perfect solution for everything from lumbar facet syndrome to chronic knee pain.

But Does It Actually Work?

We are scientists at heart, so we don’t care about nice theories if they don’t work in practice. We look at the data.

There was a major 12-month study comparing this Cryo method against standard heat ablation. The researchers wanted to see which one lasted longer. The results were startling.

At the 360-day mark—a full year later—the patients who received the Ice treatment reported a pain score of just 3.0. The patients who got the heat treatment? Their pain had crept back up to 6.1.

Even more interesting was the functionality. We measure disability using something called the Oswestry Disability Index. The Cryo group had a score of 10.1, while the heat group was sitting at 20.6.

The Ice didn’t just stop the pain; it gave people their movement back.

And regarding safety, a 24-month retrospective study of 180 patients showed zero major adverse events. It works, and it is safe.
